All students participating in a March Break, International Summers, or Community Service program must complete the following form.

If you are missing any of the required documents, please fill out as much of the Personal Information Form as you can and email the remaining documents once you receive them.

• Scanned copy of the photo and information page of the student’s passport (all students).
• Scanned copy of high school transcript (all students).
• Scanned copy of interim and final report card showing proof of pre-requisite (required for academic programs only).
• Scanned copy of Individual Education Plan (IEP) or Individual Program Plan (IPP) (if applicable to student).
• Canadian provincial health card number if you purchased medical insurance through Blyth Academy.
